          Application for Public Project Proposal
Residents or local organizations interested in submitting a public project proposal as part of a volunteer initiative should complete this application and submit it to the Clerk's office.
          Banner Display Request Application
Local organizations interested in displaying an event banner across Route 24 (Main Street) should submit this application to the Clerk's office. Approval from the Morris County Board of Chosen Freeholders and the Borough Council is required.
          Marriage/Civil Union License Application
          No Knock Registry Application
Residents can elect to include their home address on Mendham Borough's No Knock Registry. A copy of the No Knock Registry will be provided to all individuals that have applied for, and have been issued, a license allowing commercial door-to-door sales and solicitation activities in the Borough. The No Knock Registry does not apply to door-to-door solicitations conducted by charitable, religious, or political organizations.
          OPRA Request Form
Individuals wishing to obtain specific government records should submit an OPRA (Open Public Records Act) Request Form to the Clerk's office. Please be as specific as possible when identifying the record(s) you wish to obtain. Appointments can also be made for the on-site review of records.
          Peddlers & Solicitors Permit Application
Individuals wishing to conduct temporary or seasonal door-to-door or street-to-street commercial activities must submit a completed application to the Clerk's office. All required documentation, photos and the application fee must be submitted with the application form. Incomplete applications will not be processed. Licensing requires Council approval.
          Raffle License Application
Not-for-profit organizations registered with the New Jersey Local Games of Control Commission may apply for a Raffle License to conduct a merchandise or cash raffle in the Mendham Borough. Licensing requires Council and State approval.
          Road Closure Form
          Special Events Permit Application
Local organizations wishing to hold a Special Event in Mendham Borough should submit a completed application to the Clerk's office. Special Event Permits require Council approval.
          Vital Records Request
Request for certified copies of birth, death, marriage, civil union and domestic partnership records.
          Voter Registration Application
To vote, residents must be a New Jersey registered voter 30 days prior to election day. This application form can also be used to update a voter's change of name, change of address, or change of party affiliation.

          Tree Removal Application
§ 202-4 Exceptions. The following shall be exempt from the provisions of this article: A. Commercial nurseries and fruit tree orchards. B. Christmas tree plantations. C. Removal of trees which are dead, dying or diseased, or trees or specimen trees which have suffered severe damage, or any tree or specimen tree whose angle of growth or location make them a hazard to structures or human life. D. Pruning or removal of any tree or specimen tree growing on or over a public right-of-way or public property by an appropriate authority or authorized utility company for maintenance of utility wires or pipe lines. E. Homestead lots. HOMESTEAD LOT A preexisting lot located within a zone permitting single-family detached dwellings or two-family detached dwellings upon which is constructed a single-family detached dwelling or two-family detached dwelling or upon which it is proposed to construct one single-family detached dwelling or one two-family detached dwelling, and which property as a result of zoning restrictions could not be further subdivided. F. Projects which, prior to the effective date of this article, have received preliminary or final major subdivision, minor subdivision or site plan approval. Amendments to said approvals subsequent to the effective date of this article shall be subject to the provisions of this article. G. Silviculture.

          2019 Labor Day Parade Form
Here is the form to submit to participate in the 2019 Mendham Borough Labor Day Parade. Our Grand Marshal is Dotty Fleming of the Garden Club. Our parade theme is "Growing the Spirit of Mendham". The Parade will be held on Sept 2nd at 10am. This Parade Form must be returned to the Recreation Director by June 30th

P.L. 2013, Chapter 43 (Farmland Assmt – Law Changes) On April 15, 2013 Governor Christie signed S-589(4R) into law as P.L. 2013, C.43. This legislation revisees certain provision of the prior Farmland Assessment Law. This new law became effective on April 15, 2013 – except that it is applicable to Tax Years commencing with Tax Year 2015.
