• Individuals under 18 MUST be accompanied by parent/guardian.

• These Pfizer appointments are NOT for individuals ages 5-11.

Moderna and J&J will be given on Wednesdays from 3:00-5:00 pm at Saint James Church, 184 South Finley Ave, Basking Ridge, NJ 07920.

• Clinic dates for for Moderna and J&J – January 5, 12 & 19.
• Moderna primary series (first and second doses) or boosters for 18+

• Bring your original vaccination card with you if you are coming for a booster shot. If you only have a picture of your original doses, you will be given a new card. 
• No identification or insurance card required for COVID-19 vaccines.
• Please wear a mask and social distance upon arrival.
• If you are feeling unwell or are waiting on the results of a COVID-19 test, please reschedule.
